Affiliate Policies

• In many states, it is required that you disclose any affiliate commissions when making a referral. We not only recommend checking your state-specific guidelines, but encourage letting anyone you refer know that you may receive a 'thank you' gift from Rethink Nutrition for making the connection.

• Prior to receiving affiliate payouts, you'll be asked to provide Jackie Slomin Nutrition, LLC with a completed W-9 form.

• We do not accept insurance are unable to pay affiliate commissions for individuals covered by a government insurance, including, but not limited to Medicare and Medicare.
